New Patient Registration
Please complete an Application for Registration form. If your application is sucessful, please bring along your NHS medical card in order to register. It often takes time for records to be forwarded from your former practice all newly registered. All new patients will be asked to complete a health questionnaire and provide proof of identification and address.
As a guide the area includes all of the districts of Middlesborough lying within the boundaries formed by the A19 in the west, the A66 and Longlands Road to the north, Cargo Fleet Lane and Ormesby Bank in the east, the Nunthorpe Bypass in the south-east, Brass Castle Lane in the south, Hemlington Lane and Stokesley Road in the south-west. Low Lane completes the boundary to the west.
If you are on regular medication you will need to see a doctor before this can be prescribed. When making your first appointment please remind the receptionist that you are a new patient and you may be allocated a longer appointment if you feel it might be needed.
Please note that we do not accept private patients for registration. Medical treatment is available from the date of registration. Please contact reception for further information.

Named Accountable GP
The practice is required by the Government under the terms of the latest GP contract to allocate all patients a named accountable GP.
The practice does keep a record of your accountable GP. If you wish to be told the name of your Accountable GP, please ask the receptionists when you are next in the surgery.
Please note: there is no need to telephone the practice for this information.
Where a patient expresses a preference as to which GP they have been assigned, the practice will make reasonable efforts to accommodate this request.
Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor
