Extended Access


There are now Extended Access appointments for routine care available at Park Surgery, One Life.  Appointments are provided by Central Middlesbrough PCN and are available Monday to Friday 6pm - 8pm and Saturday 9am - 5pm.  If you would like to book one of these appointments please speak to a receptionist. 



NHS services across the North East & North Cumbria Region are experiencing extremely high demand at the moment – this includes GP practices and the 111 service which are both extremely busy.


If you need advice for a minor/mild illness or injury, in the first instance, please either contact your local pharmacy for help or look at the self-care advice that’s available on-line from the NHS at www.nhs.uk


To check your symptoms, there is an on-line symptom checker available at 111.nhs.uk, which will re-direct you to a health professional if you need to be consulted by someone. 


 If the surgery is closed and you require urgent medical assistance which cannot wait until the surgery re-opens, please ring 111.


 Calls to NHS 111 are free from both landlines and mobiles.


 If you have a life threatening medical emergency, please dial 999

In Times of Bereavment

If Death Occurs At Home

1. Telephone the doctor who will visit to confirm that death has taken place.

2. Contact a funeral director.

3. Arrange to collect the doctor's Medical Certificate of Death (usually from the surgery).

4. Make an appointment at the Registrars Office, remember to take the Death Certificate with you.

5. The Registrar will normally issue a Green coloured certificate for you to give to your funeral director who will look after necessary arrangements for the funeral. They will also ask you how many Certified Copies you require for dealing with the deceased finances (a fee is payable for each copy).

If The Death Occurs In Hospital

1. Contact a funeral director to inform him his services are required.

2. Collect the certificate from the hospital then follow 4 - 5 as above

Note For Cremation

Your funeral director will usually liaise directly with the surgery regarding the additional certification required.
